I think It's ok to call teachers by name because I feel at that moment that my teacher is not just here to boast "Oh, look, I'm so clever! I'm better than you, I am a teacher and you are just a student, ha ha ha!!")))) He truly is on a higher level than you are but letting you call him by name he states that he is also you friend and helper to get knowledge. This is what I feel. In my country (Ukraine) we called teachers by first and patronymic names (their father's name) which was pretty good, cause you are showing respect and good relations at the same time. I also had a chance to to to a medical college in Poland and the way they called teachers there kind of surprised me. They called them by degree. Mrs. Master, Mr. Doctor and that was really unusual and I couldn't get used to it!!!!

Yes, I think it's really fine to call a teacher by name because we also do that in my country. I guess we we call them by name because it's more formal but that's just my own opinion.

For my past we taught by parents and teacher everyone who are elder then you give them recpect.in my language i have certain words that descibe respect to elder and one lessan they taught as also never call teacher by thier first name
